You will find all the forms necessary to complete certification in your leadership designation(s) alphabetized (descendant or ascendant ordering) as individual packages of documents below.

Application Form Directions: To certify in more than one designation please fill in only ONE CFES Application Form (National Fitness Leadership Membership, Certification & Cross-Transfer)

Professional Code of Honour Form Directions: Please read, agree to and sign ONE Professional Code of Honour form for each leadership designation for which you are applying (e.g. You are certifying for Personal Trainer and Presenter. You fill out ONE application form, check off the Personal Trainer and Presenter designations on the application form, and sign the professional Codes of Honour for both Personal Trainer and Presenter).
